no checks. Payment/co-payment is due at the time
of service.

out-of-network for most plans and for Medicare.

Medicare:
We will see patients with Medicare coverage, but
do not accept Medicare assignment.You will be responsible
for submitting full payment at the time of service.
However, we will submit all charges to Medicare
and to whatever secondary insurance plans you might
have. Medicare will then send payment to you under
the terms of your coverage.
All
other insurance plans: At the time of service,
our staff will provide you with a receipt and a
Health Claim Form with all of the necessary information,
including diagnosis and service codes, required
by your insurance company to process your claim. The
Health Claim Form will then be ready for you to
mail out that same day. You will be reimbursed directly
by your insurance company under the terms of your
contract with them. Most insurance companies will
reimburse you based on an "out-of-network" fee schedule,
after deductibles have been satisfied. |
